Position: Therapy Outcomes Coordinator, Home Health – Full-Time
Location: South Carolina - Position located at any Pruitthealth Home Health Branch
POSITION SUMMARY: Direct and manage the day-to-day clinical operations of therapy services within assigned Home Health agencies. Provide clinical leadership and mentoring to PruittHealth Therapy Services Partners. Maintain effective communications with leadership and partners. Monitor, enhance and expand the rehabilitation programs within Home Health.
ESSENTIAL JOB FUNCTIONS:
Oversee all clinical and operations of the rehab department. Ensure that quality care is provided in all therapy disciplines.
Participate as directed in Quality Assurance Programs.
Attend home health agency meetings.
Coordinate recruiting and hiring of qualified Therapy Services partners.
Coordinate all required orientation/onboarding of new therapy partners.
Coordinate in-services and trainings for existing partners, including required monthly Pruitt University courses.
Complete ongoing performance discussions with partners, as well as coaching, disciplinary action, and terminations as appropriate.
Participate in marketing activities with physicians, hospitals, discharge planners, and other community organizations.
Follow established policies and procedures as related to the scope of practice.
Recognize and respond to shifting priorities in overseeing patient care with initiative and sound judgment.
Adapt to change in workload as needed.
Fosters a teamwork environment. Support cultural diversity.
Report any alleged or suspected violations to supervisor and/or others until the issue is resolved.
Performs other related duties as necessary and as directed by supervisor.
MINIMUM SKILLS AND EXPERIENCE:
Three or more years of experience as a physical therapist (PT), occupational therapist (OT), or speech language pathologist (SLP).
One or more years' experience in a supervisory capacity.
PREFERRED SKILLS AND EXPERIENCE:
- Home health experience.
LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS:
Valid professional license in state of practice.
Graduate of an program accredited by APTE, ACOTE, or ASHA in the discipline of practice.
ESSENTIAL SKILLS AND EXPERIENCE:
Ability to take initiative and use sound judgment in routine and emergency situations.
Knowledge of established safety procedures when performing tasks and/or working with equipment.
Follow established safety regulations, including fire protection and prevention, smoking regulations, infection control, etc.
Communicate clearly and concisely orally and in writing.
Develop good rapport easily with patients, families, partners and customers. Exhibit a helpful and courteous attitude at all times.
General proficiency in computer usage, especially Microsoft Office applications and electronic medical record reporting.
Knowledge of electronic medical record reporting and HIPAA regulations.
Be self-directed, prioritize well, and manage time effectively. Obtain desired outcomes and strategic goals by targeted deadlines.
Attend and participate in continuing educational programs to keep abreast of changes in the field. Maintain current license/certification, as required.
Must speak, read and write English fluently.
Be flexible. Demonstrate a strong work ethic and sense of quality. Represent the organization in a positive and professional manner.
