LPN Senior Care Partner (Nurse Navigator)
Communicatesand educates patients/residents and families on disease processes, clinicalinterventions, and expectations. Promotes customer service. Also referred to asthe Nurse Navigator.
1.Work with RN who educates our resident and/orfamily about normal aging and themultiplepathologies associated with the older person, the principles of rehabilitationand palliative care utilizing their nursing knowledge. Education will beginupon admission and be ongoing throughout the patient’s stay at thefacility.
2.Servesas liaison between the resident/family and the healthcarecenter.
3.Responsiblefor the Perfect Path process within thefacility.
4.Participates in initial and other resident careplan meetings and encourage family involvement in the meetings. Collaborateswith other disciplines and facilitates the 72-hour post admission family careconference.
5.Participatesin weekly Case Mix Meetings and ensure timely updates to familymembers.
6.Reviews 24-hour report and conducts appropriatefamily contact and follow-up. Complaints from residents and families areaddressed in a timely manner. The grievance log is maintained by the SeniorCare Partner per company policy with focus on follow up and promptsresolution.
7.Utilizes systems checklists, care guard logs,and/or quality indicator to determine residents that require interventions.Communicate with resident and family regarding the area identified,interventions implemented, and expectations and maintain frequent contact withthe family regarding theprogress.
8.Manages the Customer Satisfaction Survey processwith Family/Resident and Discharge surveys. Communicates survey results withleadership team and conducts follow up as needed.
9.As part of ongoing Customer Service, maintainscontact with resident/familyduringhospitalizations through visits, phone calls and/orcards.
10.Implementsand maintains patient scheduling program with ADLCoordinator.
11.Maintains a working knowledge of the healthcarecenters policies and procedures as well as state and federal regulations bymonitoring all current applicableregulations.
12.Must include a Registered Nurse in Care PlanMeetings for family education and anyotheractivities that do not fall within LPN scope andpractice.
Nursing Degree from an accreditedcollege or university, or is a graduate of an accredited/approved LPN program.
MINIMUM EXPERIENCE REQUIRED:
Three (3)years professional nursing experience.
MINIMUMLICENSURE/CERTIFICATION REQUIRED BY LAW:
Current, active and unrestricted LicensedPractical Nurse (LPN) licensure in state of practice.
Experience in the long-term care settingpreferred. Previous experience in quality assurance.
