Title: Care Manager AssistantLocation: Norcross, GAPOSITION SUMMARY:The Care Manager Assistant will provide a wide range of administrative and clerical support to the Care Management Department. This Partner will assist with rudimentary reporting, clinical submission, data entry, and specialized projects.ESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES: Functions as a patient advocate to ensure that all communication needs are met when reporting admissions or discharge dispositions for the continuum of care. Maintains patient confidentiality. Provides clinical updates to commercial payer in a timely manner, as defined by the payer. Possesses excellent communication skills, both oral and written. Exhibits great organizational skills. Communicates with payers and facilities regarding admissions, discharges, and clinical update requests. Attends departmental meetings and adheres to protocol. Provides reports and completes specified projects. Adheres to the Performance Improvement Program and other organizational policies. Promotes great Customer Service and Time Management Skills. Completes documentation in patient charts and in email correspondences with our customers. Participates in continuing education programs as assigned for Pruitt University. Attends and participates in any in-services or meetings for Care Management.LICENSURE, CERTIFICATION, EXPERIENCE AND EDUCATION REQUIREMENTS: RN not required Degree preferred Proficient in Microsoft Products Mulit-tasking Mon â Fri Thorough knowledge in specialized area: Hospice, Home Health, or Skilled Nursing regulations (federal, state, and local government) preferred.
