Nurse Practitioner (Hospice) – Part TimeServicing North Wilkes, NCPOSITION SUMMARY:
The Hospice Nurse Practitioner in collaboration and consultation with physicians, staff RN's and other Hospice health care professionals, provides holistic, compassionate care to patients and families and provides care coordination and care management activities on behalf of the PruittHealth Premier Plan.The Hospice Nurse Practitioner will implement Palliative Care is an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual.The Hospice Nurse Practitioner practices within the context of collaborative management with a physician(s) in diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness. Also utilizing advanced nursing practices that includes independent nursing functions based on nursing standards of care and a role in medical management working within a collaborative agreement with a physician(s).
POSITION RESPONSIBILITIES:Primary Care Delivery:1. Deliver cost-effective, quality care to assigned members2. Work collaboratively with physicians, case managers, social workers, family members, key care givers and any appropriate ancillary medical personnel as appropriate3. Responsible for identifying overall quality improvement activities through assessing and planning health care which is individualized to the member and consistent with policies and protocols4. Review and interpret the medical and social history of members5. Perform appropriate screening procedures for members6. Interpret laboratory data, recommend appropriate follow-up, reconcile medicationsManage both medical and behavioral chronic and acute conditions effectively in collaboration with a physician or specialty providerPerform comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulationsPerform Health Risk Assessment within seven (7) days of enrollment for a new beneficiary and annually within one year of the initial assessment.Conducts an annual Comprehensive Geriatric Exam that incorporates CMS Annual a Wellness Visit elements, including the Personalized Prevention Plan Services (PPPS)Ensure all diagnoses are ICD-10 coded accurately and documented appropriately to support the diagnosis at that visitProvide leadership for the care team and care team meetings, ensures the composition of the ICT is individualized according the beneficiaries, clinical and psychosocial needs, as identified by the HRAEnsure that individual plan of care is developed with specific interventions designed to meet the needs identified in the Health Risk AssessmentEstablish and document visit schedule according the HRA stratification (low, moderate, high)Ensure all Members have an ICP developed within 48 hours of HRATFacilitate agreement and implementation of the member's plan of care by engaging beneficiary, the facility staff, families/responsible parties, primary and specialty care physiciansEvaluate the effectiveness, necessity and efficiency of the plan, making revisions as neededUtilizes practice guidelines and protocols established by PruittHealth Premier PlanMay be required to participate in on-call program, after hour or coverage may be requiredTravel between care sites mandatoryCare Coordination:1. Understand the PruittHealth Premier Plan benefits, PruittHealth Premier Plan policies, procedures and articulate them effectively to providers, members and key decision-makers2. Assess the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of benefit events and communicate the process to providers and appropriate team members3. Coordinate care as member's transition through different levels of care and care settings, ensures that the ICP is communicated immediately and/or no later than 24 hours to the receiving care setting4. Conducts HRA comprehensive face-to-face assessment if the member experiences a change in condition and/or transition in care—a “trigger” event.5. Continually monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change6. Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the member's needs and wishes7. Evaluate plan of care for cost effectiveness while meeting the needs of members, families and providers to decreases high costs, poor outcomes and unnecessary hospitalizationsProgram Support and Enhancement:Regular and effective communication with internal and external parties including physicians, members, key decision-makers, nursing facilities, PruittHealth Premier PruittHealth Premier Plan staff and other provider groupsActively promote the PruittHealth Premier Plan programs in community (Skilled nursing facility, Assisted living facility, community) by partnering with key stakeholders (i.e.: internal sales function, provider relations, facility leader) to maintain and develop membership caseloadExhibit original thinking and creativity in the development of new and improved methods and approaches to concerns/issuesFunction independently and responsibly with minimal need for supervisionDemonstrate initiative in achieving individual, team and organizational goals and objectivesParticipate in PruittHealth Premier Plan and facility quality initiativesDemonstrates in-depth knowledge of Healthcare Effectiveness Data & Information Set (HEDIS)Additional responsibilities include:Establishes schedule according to approved PHP nurse practitioner workflowAddresses business and operational challengesSets team direction and leads the team appropriatelyAssists with coordination of patient admissionsParticipates in and/or directs Quality Assurance Performance Improvement (QAPI) activitiesMay oversee work activities of other supervisorsProblem solving and handling escalated issuesRotating on call responsibilitiesIs compliant with OSHA, State Board of NursingPractices in accordance with applicable local, state, and federal laws which govern confidentiality and medical information privacy regulations (HIPAA).Maintains positive control of all PHI informationPerforms other duties as assignedConsistently identifies unmet needs and gaps in care, and initiates appropriate referrals as indicated.Synthesizes information and easily diagnoses and creates a comprehensive plan of care.Achieves clinical performance targets.Accurately completes HRA and achieves target quality metric.Creates positive rapport and engagement with patients.Completes target number of HRAs for full-time staff.Participates in different roles or interdisciplinary teams as assignedAssists on special project as assigned.Participates in client meetings as requested.POSITION REQUIREMENTS:

Education: Bachelor of Science degree required, (Nursing preferred); graduate of an accredited Geriatric (preferable), Adult or Family Nurse Practitioner Master’s Degree program.2. Licensure/Certifications:a. Current RN and NP licensure in state of practice.b. Board certification by AANP or ANCC required.c. Board Certification as an Adult - Gerontology Nurse Practitioner preferred.3. Current CPR certification.4. Advanced Cardiac Life Support training/certification preferred.5. Work Experience: Minimum 1 year clinical experience as a Nurse Practitioner in geriatric, adult or family practice setting. Long term care experience strongly preferred.Preferences:Geriatric certification. If not certified, certification must be obtained within 18 months of employment.Bilingual (English & Spanish)Knowledge of community resourcesPrevious experience working with behavioral health populationsExperience working with low income and disadvantaged populationsPrevious work with vulnerable adults or senior population
